New Client Paperwork
As of January 2018, we use a secure online platform specifically created for therapy practices called TheraNest. We appreciate if you can complete the necessary paperwork digitally ahead of time before your first scheduled session so that we may review it before we meet as well as save on paper and printing. Your therapist will send you an emailed link to join the Client Portal before your first session where you can access, sign, and submit the paperwork.

If you do not have access to a computer or internet, you will need to complete paperwork the traditional way. Please print out and bring with you the following documentation for your first session. If you are unable to access a printer before your first session, please let us know and arrive 15 minutes prior to your appointment time to complete the necessary paperwork before we begin.
STEP #1:
Choose one from the following intake forms:
​ Intake Form (Child or Adolescent)
* Families - please choose from the above options and fill out as many forms as necessary for the people attending therapy
* Group Participants - forms will be handed out at the first meeting

STEP #2:
Choose the appropriate forms, print, and complete the following documents in addition to the intake form(s):
HIPAA Notice of Privacy Practices
Christie's Professional Disclosure Statement
STEP #3:
If your child is to be the client, print and complete the following documents as well:
Consent to Treat a Minor Form (not required if client(s) is/are 18 or older)
Parental Confidentiality Agreement (not required if client(s) is/are 18 or older)

STEP #4:
If you would like your therapist to collaborate with another person for your care:
Disclosure of Protected Health Information Form (not required unless you would like me to consult with other healthcare professionals about your needs or have legal issues that require a release of your confidential information)
